Health and Wellness

At Mater Dei Catholic, student safety and well-being are our highest priorities. We are dedicated to maintaining a secure, supportive environment where every child feels protected and valued. Our school nurse, Nurse McDonald, brings years of experience and expertise, ensuring students receive quality care and guidance for their health needs. Please carefully review all the information on this page, as it is essential for all students in TK through 12th grade who are planning to attend or are currently enrolled at Mater Dei Catholic.

Uploading Medical Documents: Magnus

All medical documents must be submitted through the student's Magnus Account. You access your Magnus account through your FACTS Family Portal. No documents will be accepted physically.

Immunization Requirements

Physical Exam

Students must complete an annual physical exam. Physicals must take place AFTER June 1st. Any physical done before June 1st will not be accepted and students must be re-done. The physical exam form must be uploaded to MAGNUS. No paper forms will be accepted.
