At Mater Dei Catholic, student safety and well-being are our highest priorities. We are dedicated to maintaining a secure, supportive environment where every child feels protected and valued. Our school nurse, Nurse McDonald, brings years of experience and expertise, ensuring students receive quality care and guidance for their health needs. Please carefully review all the information on this page, as it is essential for all students in TK through 12th grade who are planning to attend or are currently enrolled at Mater Dei Catholic.

Immunization Requirements
- Records needed for: Students Entering Transitional Kindergarten or Kindergarten
- Records needed for: New and Transfer Students Entering TK/K-12th Grade
- Records needed for: Students Starting 7th Grade
- Records needed for: Other Immunizations

Physical Exam
Students must complete an annual physical exam. Physicals must take place AFTER June 1st. Any physical done before June 1st will not be accepted and students must be re-done. The physical exam form must be uploaded to MAGNUS. No paper forms will be accepted.
Required Documentations
All documents must be completed and turned in online to the student's Magnus Health portal. All athletes are required to have on file an updated Physical Exam, Updated Vaccines, Sports Off-Campus Release, and Concussion forms to practice and compete with their respective team. All students must have a valid examination on file at all times (complete each year). Students not participating on a sports team are still required to have an updated physical. June 20th, 2025: On-Campus School PhysicalsAll registered Mater Dei Catholic High School students are welcome to participate in our annual school physical clinic hosted by Dr. Leticia Polanco and Dr. Trace Polanco. Clinic will be hosted on-campus on Friday, June 20th from 10:00-5:00 PM and are $30 per student. All immunization requirements must be met at the time of the examination appointment.
